MI License, MI CDS, and MI DEA required; Successful completion of an ACGME or AOA-accredited residency/fellowship or foreign training in Urology; Current certification or board eligible (American Board of Urology or American Osteopathic Board of Surgery - Urological Surgery) Required previous experience: At least 50 urological procedures, reflective of the scope of privileges requested during the past 12 months, or successful completion of an ACGME or AOA accredited residency/clinical fellowship within past 12 months.
