The incumbent of this position provides integrated outpatient, accessible health care services with their health care teams who are accountable for addressing a large majority of personal health care needs, developing a sustained partnership with patients, and practicing in the context of family and community. The care is person-centered, team-based, community-aligned, and designed to achieve better health, better care, and lower costs. The incumbent is specifically trained to provide comprehensive primary care services through residency or fellowship training in acute and chronic care settings. Conduct thorough examinations, develop differential diagnosis, and design treatment plans. Order and interpret laboratory, imaging, and other diagnostic tests. Provide counseling and education to patients on health conditions, wellness, and preventive health measures. Manage chronic illnesses such as diabetes, hypertension, and hyperlipidemia. Coordinate care with specialists, nurses, and other healthcare professionals to ensure optimal patient outcomes. Maintain accurate, timely, and complete medical records using the designated electronic health record (EHR) system.
