Key Responsibilities

• Serve as the primary physician for patients in skilled nursing and rehabilitation facilities
• Manage medically complex post‑acute and long‑term care patients, including multi‑morbidity and polypharmacy
• Conduct scheduled and responsive rounds to assess recovery, respond to acute changes, and adjust care plans
• Collaborate closely with nursing, rehabilitation, case management, and social work teams
• Communicate with patients, families, and caregivers regarding goals of care and treatment plans
• Partner with hospitalists, primary care physicians, and specialists to ensure smooth transitions of care
• Participate in quality improvement, population health, and value‑based care initiatives
• Maintain timely and accurate documentation in the electronic medical record

Qualifications

• Medical degree (MD or DO) from an accredited school of medicine.
• Board certification in Internal Medicine or Family Medicine (Geriatric Medicine certification preferred).
• Active and unrestricted medical license in California.
• Demonstrated experience in post-acute, skilled nursing, long-term care, or geriatric settings.
• Strong clinical skills in managing complex, multi-morbid patients.
• Excellent communication and interpersonal skills, with a patient- and team-centered approach.
• Ability to work effectively in an interdisciplinary, facility-based environment.
• Proficiency in electronic medical records (EMR) systems.

Preferred Qualifications

Fellowship training in Geriatric Medicine or experience in post-acute/rehabilitation medicine. Knowledge of CMS regulations, SNF billing and documentation requirements, and quality metrics.
