Clinical Care *Provide comprehensive geriatric assessments across medical, functional, cognitive, psychological, and social domains *Diagnose and manage complex, multi-morbid conditions commonly seen in older adults *Lead care for patients with frailty, dementia, falls, mobility limitations, polypharmacy, and chronic disease burden *Develop individualized care plans aligned with patient goals and values *Conduct medication reconciliation and deprescribing initiatives focused on safety and improved outcomes *Facilitate advance care planning and goals-of-care conversations *Coordinate transitions of care following hospitalization, skilled nursing facility stays, and other acute care episodes *Collaborate with caregivers and families to support long-term health and independence

Required Qualifications: *MD or DO degree *Completion of an accredited Family Medicine or Internal Medicine residency *Board Eligible or Board Certified in Family Medicine or Internal Medicine *Oregon medical license and DEA registration, or ability to obtain prior to start *Current BLS certification
