We are deeply invested in our core value to put patients first, and also deeply committed to creating a culture where every employee is valued and we learn and improve together.&nbsp;</p> <p><strong>About the Role:</strong></p> <p>We are seeking a <strong>Dedicated and Collaborative Obesity Medicine Physician</strong> to join our innovative, multidisciplinary weight management team. This new role is designed for an ABOM-certified obesity medicine physician with a strong interest in teaching and supporting Advanced Practice Providers (APPs) in providing high-quality patient care. The physician will provide direct clinical services and structured supervision for up to three APPs, helping to enhance the consistency, safety, and effectiveness of patient care across the team.</p> <h3><strong>What you will do:</strong></h3> <ul> <li>Provide direct patient care in a clinical setting, focusing on evidence-based obesity treatment and chronic weight management. The physician will collaborate with APP colleagues to co-manage a shared panel and <strong>not have their own primary patient panel</strong>.</li> <li>Review charts and conduct <strong>return visits for APP patients every third visit</strong> to ensure quality and continuity of care.</li> <li>Provide <strong>treatment planning and medical management support</strong> for complex cases or escalated concerns.</li> <li>Prescribe <strong>non-controlled obesity medications</strong> for patients under both direct care and in coordination with APPs.</li> <li>Conduct <strong>weekly 30 minute collaborative meetings and 30 min asynchronous communication time </strong>with each assigned APP to review cases, address clinical challenges, and provide ongoing mentorship.</li> <li>Ensure clinical documentation, protocols, and treatment pathways are followed consistently within the team.</li> <li>Assist in the <strong>professional development</strong> of APPs by fostering clinical growth, offering feedback, and supporting continuing education.</li> <li>Participate in team-based care initiatives and contribute to a culture of collaboration and high-quality care.</li> <li>Attend relevant departmental meetings and contribute to quality improvement efforts within the obesity medicine program.</li> </ul> <p><strong>About You</strong></p> <ul> <li>Have an MD or DO degree, are board certified and diplomates of the <strong>American Board of Obesity Medicine (ABOM)</strong></li> <li>Minimum of <strong>5 years as an attending physician in primary specialty</strong></li> <li>Active and unrestricted state license in<strong> MA or NY&nbsp;</strong></li> <li><span style="text-decoration: underline;">If you don''t have a MA or NY State License, you MUST be licensed in any of the following states ME, NH, CT, NJ, DE, MD, DC, PA&nbsp; <strong>AND&nbsp;</strong>currently holding a LOQ (Letter of Qualification) with the IMLC (Interstate Medical Licensure Compact).</span></li> <li>Have excellent interpersonal skills for patient and team interactions</li> <li>Strong interest in <strong>mentoring, teaching, and collaborating</strong> with APPs.</li> <li>Demonstrated <strong>experience working with APPs, medical residents, or trainees</strong> in a clinical academic setting.</li> <li>Excellent communication and interpersonal skills.</li> <li>Adherence to evidence-based obesity treatments, behavioral interventions, nutrition, and pharmacotherapy.</li> <li>Committed to delivering patient-centered, culturally competent care.</li> <li>Comfortable working in a multidisciplinary team including dietitians, behavioral health providers, and nursing staff.</li> <li>Are committed to the highest quality of medical care and an evidence-based approach to obesity management&nbsp;</li> </ul> <p><strong>More about Form Health’s benefits:</strong></p> <ul> <li>Competitive salary</li> <li>Comprehensive health benefits</li> <li>401k program</li> <li>Flexible work schedules and paid time off</li> <li>Paid parental leave</li> <li>Ability to participate in Form4Form, Form Health's weight loss program available to all employees and their dependents.</li> </ul> <p><strong>Form Health’s commitment to building a diverse, equitable, and inclusive work environment:</strong></p> <p>Form Health is committed to creating a culture and environment that celebrates diversity and inclusion, while fostering safety and belonging.
