You'll be part of a collaborative team delivering patient-centered care with access to system-wide resources and specialty support.

Key Responsibilities-Collaborate closely with specialists, nursing staff, and case management to ensure coordinated care and smooth transitions
-Manage acute medical conditions with evidence-based protocols and best practices
-Maintain accurate documentation and compliance with regulatory standards
-Participate in quality improvement initiatives and clinical education efforts
