Key Workflow Details ICU Model: Open ICU; hospitalists may follow patients into the ICU or can sign off via Epic. Intensivists manage all ICU admissions. Procedures: Not required; hospitalists may perform procedures if credentialed and comfortable. Admissions & Census: 14-16 census on 10‑hour rounding shifts. Dedicated admitting shifts: 6-8 admits on days/swings (10 hours) and 8-10 admits on nights (12 hours). Highly collegial support between admitters and rounders. Nights & Shifts: 2-3 swings and 2-3 nights per month; majority covered by dedicated swing admitters and nocturnists. APP Support: Supplemental APCs provide cross‑coverage only; core model is physician‑led. Workflow Tools: Strong case management and social work support. Pharmacy manages med‑rec.
