This full-time position is Monday-Friday, 9:00 AM-5:00 PM - no weekends and no on-call responsibilities - so you can spend evenings and weekends with family or on whatever recharges you. What you'll do Provide facility-based care for medically complex patients in a post-acute/inpatient setting. Work closely with interdisciplinary teams to support recovery, transitions of care, and long-term patient relationships. Practice with a high degree of clinical autonomy while having strong clinical support and resources available.
