What This Role Offers Monday Friday daytime schedule Minimal call requirements No overnight hospital rounding A consistent patient panel within LTC communities Strong interdisciplinary collaboration Administrative and documentation support While some visits are brief and focused, the overall model prioritizes continuity of care and long-term patient relationships particularly for medically complex seniors. Clinical Responsibilities Conduct admissions and new patient evaluations Manage chronic and acute medical conditions in the LTC setting Perform routine follow-ups and regulatory visits Collaborate with NPs, nursing staff, therapists, and facility leadership Communicate with families and caregivers Document encounters using a company-issued mobile device The focus is on quality, coordination, and patient-centered care within a structured post-acute environment.
