Provide clinical leadership and deliver high-quality, patient-centered care for residents of skilled nursing and long-term care facilities in the Mishawaka, Indiana area. In this full-time role you will collaborate closely with experienced Nurse Practitioners and Physician Assistants to manage post-acute and chronic conditions, oversee clinical care within assigned facilities, and support continuity and quality of care across the provider team. Key responsibilities Serve as the attending physician across local skilled nursing facilities, partnering with APPs to coordinate patient care and treatment plans. Provide clinical oversight, make medical decisions, and ensure compliance with facility protocols and regulatory standards. Conduct on-site visits (typically 18-25 visits per day) and participate in interdisciplinary team meetings. Contribute to quality improvement initiatives and communicate effectively with patients, families, and facility staff.
