We're looking for a full-time Attending Physician to provide compassionate, patient-centered care to residents in skilled nursing facilities across the Lima, Ohio area. Partner with NPs and PAs to deliver team-based, coordinated care and provide clinical oversight. Develop individualized care plans and participate in transitions of care. Engage in Chronic Care Management, Transitional Care Management, Remote Patient Monitoring, and other quality improvement efforts. Use streamlined documentation tools, including an intuitive EHR, voice dictation, AI-powered scribe technology, and UpToDate for real-time clinical support.
