Work in a team environment (Physicians, NPs, RNs) in a virtual Command Center Medically Home significantly reduces total medicine expense from avoidable inpatient visits, post-acute care and readmissions. Improved patient experience. State of the art equipment at Command Center and patients’ home. Patients have home infusion, meals/nutrient consults, pharmacy/medical mgmt., mobile imaging, laboratory and DME/oxygen. Integrated care with primary care providers, specialists & ancillary departments. Minimum Qualifications Licensure, Certification, Registration BC/BE Internal Medicine/Family Medicine/Geriatrics. Skills, Knowledge & Abilities Experience as a hospitalist or SNF a plus. Committed to providing high quality, evidenced based medicine to a diverse patient population. Excellent clinical and communication skills. Demonstrated ability to work in a team environment.
