We are seeking a PRN Physician (MD/DO) to provide coverage in three skilled nursing and long-term care facilities in the area of Murfreesboro, Cookeville, McMinnville, and Columbia, TN..
Key Responsibilities
Provide primary and transitional care services to patients in skilled nursing and long-term care facilities
Perform patient assessments, diagnoses, and treatment planning
Manage acute and chronic medical conditions
Complete required documentation and Medicare recertification visits
Collaborate with facility staff and interdisciplinary care teams
Educate patients and families regarding treatment plans and goals of care
Qualifications
MD or DO with active medical licenses in Tennessee
Experience in post-acute and long-term care preferred
Hospitalist, inpatient, or primary care experience welcomed
Strong clinical judgment and commitment to quality patient care
