This role involves providing comprehensive outpatient primary care services to patients across the lifespan, encompassing evaluation, diagnosis, and treatment of acute and chronic medical conditions, preventative care, and medication management. Responsibilities and Duties Provide comprehensive outpatient primary care services to patients across the lifespan. Evaluate, diagnose, and treat acute and chronic medical conditions. Perform preventative care, annual wellness visits, and routine health screenings. Develop and manage treatment plans, including medication management and follow-up care. Coordinate care with specialists, ancillary services, and community resources as needed. Maintain accurate and timely documentation within the EMR. Collaborate with clinic staff to support quality patient outcomes and continuity of care.
