Patients have home infusion, meals/nutrient consults, pharmacy/medical mgmt., mobile imaging, laboratory and DME/oxygen. Integrated care with primary care providers, specialists & ancillary departments. Minimum Qualifications: - Licensure, Certification, Registration: - BC/BE Internal Medicine/Family Medicine/Geriatrics. Skills, Knowledge & Abilities: Experience as a hospitalist or SNF a plus. - Committed to providing high quality, evidenced based medicine to a diverse patient population. - Excellent clinical and communication skills. - Demonstrated ability to work in a team environment.
