Full-Time Physician – Post-Acute Care

Location: Southern, NJ | Schedule: Full Time

We are seeking a compassionate and skilled full-time Post-Acute Care Physician to join our care team. In this role, you will support patients and families as they transition from hospital to post-acute settings, overseeing recovery and managing complex medical needs with clinical expertise and empathy.

About the Role

The Physician will provide comprehensive medical evaluation and management for patients in post-acute settings, including skilled nursing and rehabilitation facilities. Responsibilities include:

• Leading and coordinating post-acute medical care plans
• Managing acute and chronic conditions during recovery
• Providing symptom management and goals-of-care discussions
• Collaborating with interdisciplinary teams, including nursing, therapy, and case management
• Supporting safe transitions of care and reducing readmissions

This role offers the opportunity to build meaningful relationships with patients and families while guiding them through critical stages of recovery.

Compensation:

The starting salary for this position is: $309,000.00 (annually).
The actual compensation package could vary based on factors such as, but not limited to, the applicant’s experience, internal equity, and alignment with market data.
