Minimum Education Requirements Doctor of Medicine (MD) degree or Doctor of Osteopathic Medicine (DO) degree from an accredited medical school. Residency training program in family medicine accredited by the Accreditation Council for Graduate Medical Education (ACGME) or American Osteopathic Association (AOA). License & Certifications Board certification in family medicine by the American Board of Family Medicine (ABFM) or the AOA. Active and unrestricted medical license in New York.
