About the Role:
We are seeking Physicians specialized in Endocrinology and Obesity Medicine to join our innovative, multidisciplinary chronic disease management team, and preferably hold a LOQ (Letter of Qualification) with the IMLC (Interstate Medical Licensure Compact) if living in an IMLC eligible state. A valid medical license to practice and prescribe legally in VIRGINIA (VA), MASSACHUSETTS (MA) or NEW YORK (NY) is preferred.
This new role is Modified FTE (32 or 36hrs/week) role, designed for an American Board of Internal Medicine (ABIM) certified Endocrinologist with demonstrated expertise in obesity medicine, comprehensive diabetes management, and a strong interest in working collaboratively with Endocrine Advanced Practice Providers (APPs) in providing high-quality patient care.
A critical aspect of this role is ensuring the safe and effective management of patients with complex co-morbidities, including Chronic Kidney Disease (CKD), End-Stage Renal Disease (ESRD), and Cardiovascular conditions. The physician will help enhance the consistency, safety, and effectiveness of patient care across the team, specifically integrating the use of Continuous Glucose Monitors (CGMs) and insulin pump therapy.
This role will include participation in a rotating after hours call schedule with Form Health clinical leadership and will have a reduced clinical schedule the weeks of on-call coverage.
This is a Modified Full-Time position (W2, 32 hrs/week or 36 hrs/week), based in a remote, telemedicine clinical setting. The physician will work closely with a supportive care team and have protected time for supervision, teaching, and administrative duties.
