The Complex Care Clinic Physician delivers high-quality, person-centered, team-based primary care to a panel of 400+ medically complex, high-risk patients. This role emphasizes value-based care, population health management, chronic disease management, and care coordination across the care continuum. Care is provided in the outpatient setting via in office, home, and virtual visits. The selected physician will become a member of Summa Health Medical Group, a multi-specialty group practice of nearly 700 providers employed by Summa Health. With 2 hospitals and 20+ health centers, Summa Health provides access to care throughout the region.
