Become a part of our caring community Humana's Primary Care Organization is a provider of senior-focused, value-based care, with more than 340 centers across 15 states under the CenterWell and Conviva brands. This is a full-time, on-site, patient-facing role. You report to the Associate Medical Director and are responsible for both in-visit care and ongoing patient management. You will provide comprehensive primary care to senior patients within a value-based care model that emphasizes quality outcomes, preventive medicine, and long-term health management. What You'll Do You will see 12-15 patients per day, allowing for longer, more in-depth visits You will manage patients with complex and chronic conditions You will complete accurate, same-day documentation to support patient care and outcomes You will manage a daily clinical inbox (labs, messages, follow-ups) You will coordinate care with specialists and care team members You will apply Medicare risk adjustment principles through accurate condition documentation What You Can Expect You will work in a value-based model focused on outcomes—not volume You will see lower patient volume with higher complexity, allowing more time with patients You will collaborate in a team-based care environment supporting whole-person care You will manage care beyond the visit, including follow-up and coordination You will benefit from a supportive culture and work-life balance, including PTO, CME, and minimal call Use your skills to make an impact Required Qualifications MD or DO from an accredited program Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatrics Active, unrestricted medical license (or ability to obtain) in South Carolina Additional Experience We Value 2+ years of clinical experience with senior populations Experience with Medicare Advantage or risk adjustment Experience supervising Advanced Practice Providers (NPs/PAs) Proficiency with electronic health records (EHR) and digital documentation. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $255,000 - $298,500 per year This job is eligible for a bonus incentive plan.
